Requirements & Forms

Physical Exams

• Required for Kindergarten, 6th grade, and new admissions.
• Prefer completion between June 1–September for the coming school year.
• Send completed forms in with your student (email submissions may be unreliable).

Immunizations

• Must be up to date per the Commonwealth of Pennsylvania requirements.
• Non-compliance may result in exclusion from school until verified by a doctor’s note.
• A state immunization card must be on file.

Health Information (S-865)

• Required annually for every student.
• Report conditions such as asthma, diabetes, seizures, ADHD, cardiac issues, etc.
• Inform all teachers of health concerns that could impact performance.

Medications

• All prescription and OTC medications require a doctor’s order.
• Nurse may administer Tylenol, Motrin, and hydrocortisone with parent/guardian consent on the S-865.
• All other medications: bring in original, labeled pharmacy container; parent/guardian must deliver to the nurse (students may not carry them).
• Grades 4–8: Students may self-carry inhalers and EpiPens with explicit doctor authorization and completed forms.
• If no doctor’s order is on file, a parent/guardian may come to school to administer the medication.
• Pick up all medications at year’s end; unclaimed medications will be disposed of properly.

Illness, Injuries & Gym

• Nurse has set office hours for routine visits; emergencies are seen anytime.
• Do not send children to school when sick. The nurse cannot diagnose or treat injuries/illnesses that occur outside school hours or off school grounds.
• Absences: If absent 3+ days, provide a doctor’s note to the teacher. Excessive absences require physician documentation.
• Gym: Doctor’s note required for injury/surgery restrictions and a clearance note to return; without clearance, students may not participate.

Health Screenings & Prevention

• State-mandated screenings: vision, hearing, height, weight, scoliosis (performed by school nurse or approved provider).
• Opt-out: Send a written letter by the second Monday in September and provide a physical showing that these assessments were completed by a physician. Without a letter, screenings will occur at school.
• Encourage healthy routines: good hygiene, adequate sleep, breakfast, and staying home when contagious.

• MED 1 (as applicable): Asthma, Seizure, Any Medication, and/or use of suction, oxygen, or other equipment in school
• Medication forms must accompany any medication when delivered to the nurse.
