  To: Parents/Guardians:
  
  I hereby opt into Continuous Enrollment. This means that my child(ren) will continue their education at St. Cecilia Catholic School until graduation from eighth grade.
  
  I understand that I have the option each year to notify the Main Office of any changes in my enrollment plans for my child(ren) by completing an Opt-Out/Withdrawal Form.
  
  I understand that all enrollment and financial agreements signed for this school year will apply to all years going forward.
  
  I understand that a FACTS account must be established with a tuition payment plan option.
  
  Once your FACTS account is set up, the agreement will be the same each year unless written notice is provided to the Tuition Office.
  
  I understand that the Continuous Enrollment fee is billed annually through my FACTS payment plan.
  
  I agree to keep our child(ren)'s demographic information updated in the school's information system (FACTS SIS) and FACTS by promptly notifying the school's Main Office of any changes.

  I agree to review the SCS Handbook each year for any changes to these policies.
  
  I agree to fulfill all financial obligations promptly.
  
  I understand that each year, St. Cecilia Catholic School will communicate via email the following school year's tuition rates and post it to the website.
